Coxarthrosis (Hip Joint Deterioration)

Cartilage degeneration of the hip joint: pelvic, groin, and thigh pain, limping, and restricted movement. Proper and timely care preserves joint function.

What is it?

Coxarthrosis is a chronic degenerative condition characterized by the gradual breakdown and thinning of the hip joint's cartilage tissue, accompanied by the deformation of joint surfaces. The hip joint is one of the largest weight-bearing joints in the human body, supporting the weight of the upper body and enabling key functional movements such as walking, sitting, and bending.

As the condition advances, the smooth protective cartilage wears away, synovial fluid decreases, and the femoral head rubs directly against the acetabulum (hip socket). This friction causes bone spurs (osteophytes) to form along the bone edges, narrowing the joint space. Consequently, it leads to chronic pain in the thigh and groin, shortening of the femur, and noticeable limping. Early detection and proactive care help prevent the need for joint replacement surgery (hip endoprosthetics).

Main symptoms

  • Groin and Thigh Pain: Pain predominantly focuses in the groin, anterior, and lateral thigh, occasionally radiating down into the glutes or knee.
  • Movement-Induced Pain: Discomfort significantly increases during weight-bearing activities like walking, standing up, or climbing stairs.
  • Limited Rotational Mobility: Rotating the leg inward or outward, bending, putting on socks or shoes, clipping toenails, and sitting cross-legged become difficult.
  • Limping: Inability to place full weight on the affected leg results in an involuntary, protective limp.
  • "Start-Up" Stiffness: Sensation of heaviness and joint tightness during the initial steps after sitting in one position for an extended period.
  • Muscle Weakness (Atrophy): Due to pain-induced inactivity, the thigh and gluteal muscles begin to atrophy and lose strength.

Causes and risk factors

  • Age Factor: After age 40–50, the natural regenerative capacity of cartilage diminishes, especially when paired with an unhealthy lifestyle.
  • Congenital Defects (Dysplasia): Underdeveloped hip joints from birth or shallow acetabular sockets.
  • Excess Body Weight: Every extra kilogram significantly amplifies mechanical load on the femoral head, speeding up cartilage wear and tear.
  • Injuries and Trauma: Fractures of the femoral neck, dislocations, or repetitive micro-trauma.
  • Impaired Blood Circulation: Disrupted microcirculation in the femoral head deprives joint tissues of essential nutrients.
  • Chronic Heavy Strain: Strenuous physical labor, prolonged standing, or high-impact athletic activities.

Prevention and self-care

  1. Maintain a Healthy Body Weight

Weight reduction considerably relieves continuous mechanical stress on the hip joint.

  2. Choose Low-Impact Exercises

Swimming, therapeutic physical therapy (PT), and aquatic exercise enhance mobility without pounding the joints.

  3. Use Supportive Mobility Aids

During flare-ups, using a walking cane temporarily protects the joint from overload.

  4. Avoid Prolonged Static Postures

Do not stay seated or standing continuously; shift posture and gently move every 30–40 minutes.

  5. Strengthen Surrounding Muscles

Cautiously building up the gluteal and thigh muscles helps stabilize the joint along its natural axis.

  6. Start Nutritional Care Early

Begin taking chondroprotective supplements in a timely manner to nourish and support cartilage structure.
